What is Health Care Share? In partnership with the Vermont Youth Conversation Corps (VYCC), we connect our patients to fresh, organic food grown at the VYCC farm, powered by youth engagement. With 165 weekly farm shares, we reach over 400 household members each season.

How does it work? CVMC providers and Community Health Team members enroll patients experiencing food insecurity, one or more diet-related illnesses, or other distinct health risks who pick up weekly shares in Berlin, Barre, Northfield, Montpelier and Waterbury.

When does it run? From June to November, the program supports our patients and their families to eat more vegetables and healthy proteins, reduce medications, try new foods and recipes, and achieve better health outcomes.

How can I help? The CVMC Health Care Share program is funded entirely by philanthropic support. You can help deliver a season of fresh food to CVMC patients.
