To sign up using the online web form and receive your activation email, go to:
MyChart.UVMHealth.orf/MyChart/signup .
To use an account activation code received from your medical provide to use the code at the bottom of your printed After Visit Summary, go to:
MyChart.UVMHealth.org/MyChart/accesscheck.asp
You do not need to create a new MyChart account if you already have a UVM Medical Center MyHealth Online account. Your username and password will work in MyChart.
Submit Your Information
Fill out the secure account form and click Submit request.
Check Your Email
Upon submission, you will be sent an activation link by email. Clicking this link from your email returns you to MyChart and inputs the code automatically.
Activate Your Account
Confirm your birthday and click Next.
Choose a Username & Password
Fill out the account information page with your desired username, password, and security question. Accept the terms and conditions by clicking Accept at the bottom of your screen.
Update Your Communications
Choose your preferred communication methods for MyChart communications and alerts.
Complete Two-Factor Authentication
Complete an additional security step the first time you log on and any time you log on from a new device. You’ll be asked to enter your email to receive a security code, which is then entered in MyChart.
What is MyChart?
MyChart is the online patient portal associated with Epic, the electronic health record (EHR) used by the UVM Health Network.
An EHR is a real-time, digital version of a patient’s medical record and can be viewed and managed by authorized health providers. EHRs have patient portals that give patients 24/7 online access to their personal health information from anywhere with an Internet connection.
The Network is implementing Epic and MyChart at CVMC in two phases:
- November 9, 2019: Ambulatory departments and outpatient practices (see question 2).
- October 9, 2020: Hospital inpatient services, which often involve hospital stays.
Epic and MyChart also launched at Porter Medical Center and Champlain Valley Physicians Hospital on November 9. The UVM Medical Center already uses Epic and their patient portal, called MyHealth Online, was upgraded and renamed MyChart on November 9.
Which practices at CVMC will be getting MyChart for their patients?
MyChart became available to patients of most CVMC ambulatory departments and outpatient practices on November 9, 2019.

- Radiation Oncology
- Day Hospital/Infusion Room
- Dialysis locations
- All Rehabilitation Therapy locations
MyChart is NOT replacing CVMC’s inpatient health portal—Meditech Central to Your Well Being—at this time. Patients receiving inpatient services, which often involve hospital stays, will maintain access to the existing inpatient portal until December 31, 2019.
How long will I have access to my existing outpatient portal powered by eClinicalWorks?
Patients can still access the old eClincalWorks portal, but any information after November 9, 2019 can only be found in MyChart.
Will anything change regarding how I view or pay my bills?
No. Patients will continue to pay their bills online using the online e-billing service:

What information from my existing outpatient portal (eClinicalWorks) is visible in MyChart?
The following information is visible in MyChart:
- General and microbiology lab results going back 3 years
- Pathology and cytology lab results going back 10 years
- Imaging results interpretation for radiology and cardiology going back 5 years
- Future appointments
- Current problem list, allergies, medications, and immunizations. Please note, data will not be fully available on November 9.
- Care team members for primary care and those linked to appointments in the next 90 days.
